  • Cash or Check:
    We gladly accept cash or check at the time of service.
  • Credit Cards:
    We will gladly accept payment by MasterCard, Visa, Discover and American Express.
  • Financing:
    Deferred interest, installment loans are available through Care Credit. Approval is usually available within 30 minutes.

Accepted Insurance

As a courtesy to our patients we will be happy to assist you with your insurance paperwork and submit them for you. Our office is committed to helping you maximize your insurance benefits. We will submit pre-determination of benefits and will accept assignment with your payment of the uninsured portion.

Insurance companies do vary on their coverage. We can only ESTIMATE your coverage in good faith but cannot guarantee coverage. Patients are directly responsible for all charges, regardless of decisions made by their insurance company. The insurance policy is a contract between you, your employer and your insurance company; we make no guarantee of any coverage or payments from your insurance provider. Patients must be aware of their exclusions, maximums, required co-payments and effective date of coverage. The amount insurance pays is predetermined by your employer and insurance company.

We will do our best to help you but most questions are best answered by your employer. For patients with whom we have verified eligibility with an insurance provider, we are happy to file the necessary forms with your insurance company to enable you to receive your maximum possible benefits. You are ultimately responsible for all charges regardless of insurance.

We accept co-pay and other uninsured portion at the time of service and we will bill insurance companies allowing 14 days for payment. After 30 days, you are responsible for the entire balance.

Terms and conditions:

Our office will make every effort to accept your appointment request times and dates along with your preferred doctor or hygienist but we cannot guarantee availability. Your appointment request is not confirmed until you hear from someone in our office. A broken appointment is a loss to three people; the patient who missed the valuable time, the patient who could have used the valuable time and the doctor who was fully staffed and prepared for your visit. If you must cancel an appointment, please provide us with a 24-48 hour notice. Thank you!
